The bloated neck of George Lucas

I find George's neck to be a distraction and unnerving. I'm afraid if I were near him I'd be overcome with the temptation to poke it.

Is it filled with fat? It appears to be a skin balloon filled with air but that wouldn't make sense anatomically.

Is this something he could have fixed surgically? If so, why wouldn't he? Why did his neck skin just up and fall down?

I cannot fathom being obscenely wealthy and not fixing such a strange body feature.
