In my experience, a heated mattress pad has been best.
Because heat rises, it naturally warms sheets and blankets above it, making it very effective for pre-warming the bed. For that purpose, though, you need a simpler model, i.e., one that doesn't turn off the heat when it "knows" that the bed is unoccupied.

No thanks to either option. I prefer sleeping in a cold room with a good comforter or quilt to keep me warm.

Heated mattress pad. Discovered them about five or six years ago; nothing's better. Cold room, warm bed, great night's sleep.
Even auto shut-off after a certain number of hours in case you forget in the morning. Ten levels of warmth. On 10 it's like a heating pad for sore muscles.
Wonderful invention - why heat the room instead of the bed?

I've tried both and found I prefer the blanket (Blanket With A Brain, in particular) because I can get away from the heat quickly. The mattress pad is there, no matter if you shut it off, you are still in its heat until it cools down. Blankets can be pushed away and you can cool down as quick as you want. Don't get me wrong, I love the heat on my back, but the pad doesn't work for me.
